Trevor Leventhall, 25, of Wake Forest, North Carolina, was found dead in the upstairs bedroom of 206 Wall Ave on October 30, 2018. Officers were dispatched after a 911 caller reported a woman screaming in a yard about a dead man; a handgun was found next to Leventhall and he was pronounced dead at WakeMed from a gunshot wound to the head. The search warrant application did not specify how many times he had been shot.

Police identified the woman who had been screaming as Abby Seavert and believe she and Leventhall lived together and were the only people present in the house when the incident occurred. Drug paraphernalia was also located in the residence.
