A five-year-old boy, Savier Jones, died in Avondale, Arizona, from a self-inflicted gunshot wound that occurred inside his home while he was with his mother. Authorities described the shooting as accidental and found the handgun Savier accessed belonged to his father, Michael Jones. Investigators say the firearm had been left loaded and was accessible to the child; Michael Jones was charged with manslaughter and child abuse for recklessly causing his son’s death.

Records show the Department of Child Safety had earlier contact with the family: neglect reports involving Savier and his siblings were made in 2011 and 2012, the second of which was substantiated though the family declined offered services at that time. After Savier’s death the department provided services and the other children were assessed as safe under a safety plan.

Does anyone in the Avondale area have information about the family or the events leading up to this tragedy?
