A training exercise in Robinsonville, Mississippi, on January 21, 2015, ended with the death of John Ballard Gorman, the Mississippi Gaming Commission’s Director of Investigations. Gorman, 45, was role-playing as an assailant armed with a bladed weapon at about 9:00 a.m. inside the commission’s office when another agent’s firearm discharged and a live round struck Gorman in the chest. He was pronounced dead at the scene.

Gorman had been with the commission for 22 years and had celebrated a promotion to Director of Investigations the day before. The Mississippi Bureau of Investigation led the inquiry; the death was officially ruled an accident. No arrests or criminal charges were filed. The agent whose weapon fired was placed on administrative leave with pay while the investigation was pending. Gorman is survived by his wife and two daughters.
