Officer Jared J. Forsyth, a 33-year-old white male, tragically passed away on April 6, 2015, in Ocala, Florida. Jared Forsyth was an officer with the Ocala Police Department and was participating in a firearms training exercise at the Lowell Correctional Institution's firing range when the incident occurred. During the training, a firearm was accidentally discharged, and the bullet struck Officer Forsyth in the side of his chest, between the panels of his bulletproof vest.
Despite being rushed to Ocala Regional Medical Center and undergoing surgery, Officer Forsyth succumbed to his injuries approximately two hours later.
The Florida Department of Law Enforcement (FDLE) conducted an investigation into the accidental shooting. The investigation revealed that another officer was unloading his .40-caliber Glock when the weapon accidentally discharged. The bullet ricocheted off a bench and struck Officer Forsyth in the arm, traveling through his arm and into his side and chest. The investigation concluded that the shooting was accidental, and no criminal charges were filed against the officer involved.
