On December 30, 2023, at approximately 4:30 p.m., authorities responded to a reported assault with a weapon at Dignity Health Urgent Care on Sierra Lakes Parkway in Fontana, California. Upon arrival, they discovered 20-year-old Tyler Monfort seated inside a parked vehicle, suffering from a gunshot wound.
