Carolyn Velasquez was 15 when she disappeared from Ventura, California, on September 18, 1991. She walked away from a group home with no known destination and was never heard from again; her disappearance was reported to the Ventura Police Department the same day.

At the time she was 5 feet 4 inches tall, about 150 pounds, with mid-back length brown hair and brown eyes. Distinctive tattoos reported were two unknown designs on her left hand and another unknown tattoo on her left ankle. What she was wearing and whether she had any belongings when she left the group home have not been disclosed.

The lack of witnesses or confirmed movements after she left makes the case difficult to resolve and it remains open. Does anyone have information about Carolyn, the group home she left, or remembers seeing her in Ventura around September 18, 1991?
